Incident Report
The following form is a recommended form for use in club or at events in case of an injury, incident or otherwise to record the particulars. Such forms may be submitted to your PTSO or Archery Canada for filing. It is recommended to store such files for up to 10 years.

Insurance Certificate Requests
Clubs that are registered with their Provincial or Territorial Association (except Manitoba and Quebec) may request a Certificate for Proof of Insurance or name an additional insured by completing the attached form below and returning it to insurance@archerycanada.ca

Olympic Games).
